Office Policies
Payment Policy

Insurance: Please note that we do not currently accept insurance or participate with Medicare, Medicaid or HMOs. This decision was made so that we can dedicate all of our attention to providing individualized care for our patients.
Payments are due in full at the time of service by credit card, CareCredit or cash. Please note that our system does not support contactless pay methods at this time.

After your visit, we will provide a superbill which you may submit to your insurance directly for possible reimbursement.
Our services may also qualify for HSA expense reimbursements. We recommend checking the most current eligible HSA expense list to confirm eligibility.

Telehealth Visits
​
We offer in-person and Telehealth visits for your convenience. Please note that all patients requesting care, including Telehealth visits, must be seen in-person at least once every 12 months.
HIPAA Privacy Policy
​
Prior to your first appointment, you will receive a copy of our HIPAA Privacy Policy and Acknowledgement of Receipt by email. You may also download a copy of our policy here.
Cancellation Policy
​
Please keep in mind that your appointment time is reserved in advance for you. We understand that life happens and you might need to cancel. Please try to give us advanced notice if possible.
Cancellations with less than 24 hours notice may incur a cancellation fee.
Office Staff Availability
​
During our office hours, we will do our best to respond to all of your concerns. After office hours, we will be available once we return to the office. If there is ever a life-threatening emergency, please go to the nearest emergency room immediately.
